Check Your Eligibility

Eagle Guard supports NDIS participants, My Aged Care recipients, and private clients. If you have an active NDIS plan or a My Aged Care referral code, you are eligible to access our services.

Make a Referral

You, a family member, your support coordinator, or a healthcare professional can submit a referral on your behalf. Use our online referral form or call us directly on (02) 9000 0000.

Initial Assessment

One of our qualified practitioners will contact you within 2 business days to arrange an initial assessment. This helps us understand your goals and design a personalised support plan.

Service Agreement

Before services begin, we will provide a Service Agreement outlining the supports to be delivered, the schedule, costs, and your rights. You have the right to review and negotiate this agreement.

Services Begin

Once the agreement is signed, your supports commence. Your dedicated support worker will be introduced and a schedule confirmed with you or your representative.

Funding

Funding & Pricing

Eagle Guard is a registered NDIS provider and an approved My Aged Care service provider. We charge in accordance with the NDIS Pricing Arrangements and Price Limits.

NDIS Funded

If you are self-managed or plan-managed, we can invoice you or your plan manager directly. Agency-managed participants can access our services through the NDIS portal. All prices align with the current NDIS Price Guide.

My Aged Care

We accept Home Care Package (HCP) funding at all levels. Contact your My Aged Care coordinator to add Eagle Guard as a service provider on your package.

Private / Out-of-Pocket

Private clients are welcome. We will provide a clear quote before services begin. Payment is due within 14 days of invoice. We accept bank transfer and major credit cards.

For a personalised quote or to discuss your funding options, please contact our team. We are happy to work with your support coordinator or case manager.

Agreement

Service Agreement

A Service Agreement is a document between you and Eagle Guard that sets out the supports we will deliver and how we will work together.

Your Service Agreement includes:

  • The types and frequency of supports to be provided
  • The cost of each support and how payment will be made
  • How to change or end the agreement
  • How we will manage feedback, complaints, and disputes
  • Your rights and responsibilities as a participant
  • Our obligations as a registered NDIS provider
Your Right to Exit

You are never locked in. You may end or modify your Service Agreement at any time with reasonable notice. We will always act in your best interests.

Rights

Your Rights & Responsibilities

Eagle Guard is committed to upholding the rights of every person we support. These rights are protected under the NDIS Code of Conduct and the Aged Care Quality Standards.

Dignity & Respect

You have the right to be treated with dignity, respect, and courtesy at all times, free from abuse, neglect, and exploitation.

Choice & Control

You have the right to make decisions about your own life, including which services you receive, when, and how they are delivered.

Privacy & Confidentiality

Your personal information is protected under the Privacy Act 1988. We will never share your information without your consent, except as required by law.

Feedback & Complaints

You have the right to provide feedback or make a complaint without fear of reprisal. We have a formal complaints process and will respond within 5 business days.

Safe & Quality Supports

You have the right to receive safe, high-quality supports delivered by qualified, screened, and trained staff.

Advocacy

You have the right to have a support person, advocate, or representative present at any meeting or assessment.

Your Responsibilities

  • Treat Eagle Guard staff with respect and courtesy
  • Provide accurate information to help us deliver the right supports
  • Give reasonable notice if you need to cancel or reschedule a support
  • Notify us of any changes to your NDIS plan or funding

We welcome all feedback — positive or negative — as it helps us improve. If you are not satisfied with our response, you may contact the NDIS Quality and Safeguards Commission on 1800 035 544 or the Aged Care Quality and Safety Commission on 1800 951 822.
